My account was suspended for putting a credit card number question in the form. I corrected it but I'm getting an error when I try to submit it to you for reactivation. There is an error saying: "There is an error with the identifying credentials for the form you are trying to submit. If you are the owner of this form you need to make sure to paste the html for the form exactly as it has been provided".

I copied the corrected html text and copied it into the box asking what I have done to correct the problem.
