The "Reset" and "Submit" buttons do nothing.
I could "Print", but I really, REALLY need this form to work.
What should I do? I have tried everything and I can't even find any customer service number, nor does this site have a way to send issues.

This is likely an issue with either your web host not allowing forms, or an issue with the code surrounding the myContactForm.com form. If you could post a URL so that I may look at the code on your site, I'll be able to further diagnose the issue.
