Issue 1: When the form is displayed on my site, an error message appears:

'Error: upgrade your account'


Issue 2: When trying to click on my forms on the mycontactforms website the following message appears:

The requested URL /panel/appetitobar.com/appetitobarlondon5dec272010_006.htm was not found on this server.

Please help.

The first error message is there because once you allowed your account upgrade to lapse, visual verification (captcha) becomes disabled. To re-enable it login and click 'Form Security / Anti-SPAM' and reenable the visual verification. Ignore the second error.
