Can I create a template in order to create dozens of forms conatining the same information but different URL's?

You have a couple options.

1. Use the 'Copy' function under the 'Form Management' panel to duplicate your forms.

2. Use a single form and list all the URLs it will live on under Edit > Basic Form Information > Form Location
