The form I have is giving the same information on 2 lines but should be giving different info.

Realtime example:

"Name" and "School Name" below both say "Candi Norris" School name is entered differently on form. "School Name" should be different. I have deleted and re added the line, it didn't change anything.

Please Advise!!!

Please go into the administration panel and delete both of the questions that are affected by the error. Once you do this, re-add them, and update the HTML code on your website. If this doesn't work, you'll likely need to rebuild the form.
