The test page that is horribly formatted is

I want the table to be 600 pixels wide, with the left column that contains questions to be 200 pixels wide. The right column of the table containing the fields for the answers should be 400 pixels wide.

And the table should be contained within the div where you see the explanatory text.

Instead, the table stretches across the width of the entire 1000 pixel outerwrapper width.

When I view this in code and design view in dreamweaver cs4, the form is over 2000 pixels wide!

How to do I get this to format correctly?

Good Day,

The reason that your formatting is being stretched is because the question 'Share Your Story of Healing' has been specified to have a very large width (400). Please set this to something smaller (like the 40 you used on the other fields) and it should fit within your layout without stretching it.
