1. Login
2. Run through the Form Creation Wizard. The Job Application form is one of the pre-defined templates.
3. Upgrade the account to Premium (under the Premium Membership portion of the control panel). This enables the upload field option and verification code capabilities.
4. Click 'Xtras' to enable an upload field for the form you created in #2 above.
5. Click 'Form Security / Anti-SPAM' and enable Visual Verification / CAPTCHA for your form.

If i upgrade to premium accout for 1 year, will i get this service (Job application form)for life time or every year i have to renew?? |

You would need to renew each year in order to retain the upload field and visual verification.
