I have used a sample form to start to develop my own form but once I reach 27 questions, the next questons delete the question number one and the following delete questin number 2.

In a nutshell, can I not write a bigger form, let us say 40 questions?

The form can have unlimited fields. Please try deleting the form you are having issues with and then starting from scratch. Also try limiting the number of questions added to the wizard, and then adding more individually once the form is create (Edit > Form Questions > Add Question).
