when my form gets filled out and sent it comes to me with "checkbox273333" in the "Contact Me By" checkbox. it is just a phone or email checkbox. i use the same form on other sites with no problem. with this one site it will not work no matter what i do. have even copied a form i know works and put it on this site and still the same results

Delete the questions with the issue from the admin panel, and then re-add them. If the problem persists, delete the form and start over.
