After the form has been submitted and I receive the email in HTML form. This, "checkbox322406", is appearing next the the questions where I have a "yes" or "no" check box.
I can't tell if the person actually checked yes or no.

Here is the link to the form:

The form name is called "Returning Staff Application" in my files.

Please advise.

Please delete the question with the error and then re-add it. If this doesn't solve the issue, you'll need to rebuild the form.

we're experiencing the same exact problem...each time i rebuild it comes back. any ideas?
