The following is a sample of how the form is being received
in my E-mail's inbox:

Your Last Name : c
Your Real Age : e

Information that is input...including the Message field, are all appearing as single letters. _ I've tried sending the form to myself, so that's how I know the form ain't workin' right. _
It worked perfectly before 07 / 24 / 2009.

What is the URL of the form?

You will probably need to delete and rebuild the form within the administration panel in order to resolve the issue.
