When I export it doesn't tell me which checkbox was chosen during the submission. They all say “checkbox301720” instead of “Elder, Deacon, Trustee” etc. Even when I manual view the entries through mycontactform it says the same thing... I looked at the coding and can not see a difference there either...

There is an error in the database record for the form. This is something we're working on fixing in the next update, but to correct it you'll need to rebuild the form with the issue.
