The problem is that you have not set the redirect page to a valid webpage. This is why you are getting the Error 404 once the form is submitted.

To solve this:

1. Login
2. Click 'Edit'
3. Click 'Redirect Page'
4. Specify a valid URL for the user to be taken to once the form is submitted
