Updated form fields, copied HTML and reuploaded, no update!

I changed one of the form fields in one of my forms, adding an additional drop down menu item. I copied the new HTML, pasted it on my web page, and uploaded it to my server....but it didn't update.

Any idea why? When I preview the form in my browser (I use DreamWeaver) it IS updated...but when I look at it live, it's not. Help!
