I'm having problems with some of my forms but not all of them. I have three stores that need the same two forms sent to them. I have made two forms and pages for each store. One set of forms work with no problem(the first set I made) the rest of the forms will not work. Here are the url's to the pages that do NOT work

This is the error message I get when submitting those pages
This form is not being sent from the url where it is supposed to be located. If you are the owner of this form you need to ensure that the 'Form Location' is specified as portraitavenue.com/fairfield-scheduling.html. To change this, login to the site, click 'edit' next to the form's name, and then click the 'Basic Information' link to change the form's location.

Please help, not sure what else to do.

Make sure to do what the error message says. Add all versions of the URL where the form will live into the configuration. When people have multiple copies of the same form, they often confuse one form for another and do not enter the proper urls into the config.
Please go through form by form, enter the URLs into the config (Edit > Basic Form Information > Form Location), and then regenerate and paste the HTML.
