On may 15, 2008 i updated my password and received a confirmation e-mail. Now when i return to form management i find that my form is no longer available. Is there a way that i can send the form to you and have it restored in your database? thanks

There is no way to restore a form that has been deleted. This would of had to have been done by someone with access to your account.
