I get the following error:
This form is not being sent from the url where it is supposed to be located. If you are the owner of this form you need to ensure that the 'Form Location' is specified as b1sol.com/contact/sapb1cdrequest.htm. To change this, login to the site, click 'edit' next to the form's name, and then click the 'Basic Information' link to change the form's location.

I have reset the link to be : http://www.b1sol.com/contact/sapb1cdrequest.htm

which exists as a form on my website.

I have re copied & pasted the html.
What could be wrong?

Your code is incorrect.

The code you have listed on your site has your username listed as: stellar@b1sol.com. The code should reflect your actual username of: Ianf@b1sol.com. It looks like you probably changed your username and then did not update the HTML code. Login to the account 'Ianf@b1sol.com' and copy the code from there, and it should work.
