I was having problems with my survey, so I rebuilt it and it's working fine. Unfortunately, there is no folder associated with the new survey in my message center. None of the new messages are showing up.

I forgot to mention that I did not delete the original (broken) survey. I just built a new one with a similar name.

Please try deleting the newly created form, recreate it, and see if the folder is created.
