I have added the html code to my site and when I preview my site on the web the form is in a table format and is not functional.

My site was made by somebofy else and I use a content management system to upload information. I added the forms html code in the html source editor.

Please let me know what can be done to make the form compatible with my site.

Unfortunately, it is not possible for me to provide you with assistance as I do not have access to your CMS and have no understanding of the way that the CMS handles html. Our forms are written in standard HTML and should work on any platform. You will need to contact the author of your CMS in order to figure out how to get this to work.
