How do you put headings in without having a text box, check box, etc.

I want headings at the top of different sections of the form, and I can't figure out how to put them in.

Also, spacing between sections. How is that done?

There is no capability to do something like this from the myContactForm.com user interface. Instead, you will have to do it by hand using a html editor such as Dreamweaver or Frontpage.
