in my form management, it's called Dialysis Meals dot net.

I checked that it is sending from the correct page. I got the new html and pasted it, that does not work either.

Right now it's in a bootstrap layout, but as I said, copying the table as is doesn't work, either (I did add this page):

It looks like the code on the site is correct. Without being able to do too much testing, my guess is that some of the javascript / jQuery code your site is using is disrupting the submit button functionality. I would attempt removing bits of the javascript, one piece at a time, and testing. This should give you an idea of where the conflict is, so that it can be fixed.
