CONTROL PANEL/FORM MANAGEMENT/Have a Bounce - DEAL 1/2

Problem with these three fields:

SELECT DELIVERY POSTCODE AREA:(radio buttons)
DELIVERY TIMES: (dropdown menu)
COLLECTION TIMES: (dropdown menu)

All these 3 fields has got options to choose.
My customers fill out those fields and I have checked and fill out the booking form myself as well . But all information comes BLANK into my email so i have no idea what options has been clicked. Thats nnvery bad as i`m in the middle of the season.
Could you please help me with this issue. Many thanks

I would recommend deleting these questions and then re-adding them to the form and updating the code. If this doesn't solve the issue, I would recommend rebuilding the form.
