I keep receiving the following error - no matter what email address I use:

Error!

# We're sorry but you did not fill out all of the required fields. Please push the back button on your browser and fill out all of the required fields.

- You need to enter: Email Address

I have copied and pasted directly into Composer and GoLive! and still get the same result.

It appears that when you created the form you created a question called "Email Address". This was a duplicate because an e-mail address field is automatically added to all forms. When you saw that you had two email address fields you simply deleted the extra field from the html for your form and uploaded it to your web server. The problem is that you haven't deleted it out of the online control panel. Our systems are expecting to see the extra email address field that you added and, since when you created it you specified it as being required, you are getting the error message.

Here is what you need to do to fix the problem:

1. Login to the site
2. Follow the [edit] link next to the form
3. Click the link that says Form Questions
4. Click the Delete button next to where it says Email Address (listed under form questions)
5. Re-copy and paste the html to your site
