How do i provide my host proivder a link to my forms? I gave him 2 url links already while in html and preview form..but that is not working. ?

You have two options:

1. Provide them with the form HTML from the 'Get HTML' page and they can build the page using the HTML we've provided.

2. Click 'Xtras' and then 'Hosted Form' which will provide you with a live public URL for the form.
