I pasted the HTML for my first contact form into the body of my Blogger blog post but instead of appearing as a contact form in the post, the fields appear very messed up all over the header of the blog.

This appears to be a problem with the way the blogging software is interpreting the code. Unfortunately I am unfamiliar with Blogger's interface, so you will need to contact them for support regarding this.
