Why is the script rendering like this on my page: http://www.complocal.com/?page_id=24

I copied all the HTML code as required.

It appears okay to me...did you modify the form HTML?

Two fields to me appaear to the right of the main form table:

1) If so what domain name would you like?:
2) Is Digital Photography Required? :

I have checked this in both IE8, Firefox & Chrome.

I can send you a screenshot if you need?
