We would like to create 10 forms consisting of 2 text boxes in each form.
In the first first box the participant enters the "event" and in the second box the user enters multiple results from the event.

myContact easily created the simple form but we cant seem to work out how to create the pairs of text boxes.

You would need to manually manipulate the exported HTML within an external HTML editor in order to achieve this look.
