I am trying to get this to work, I've used the wizard and reconstructed the form several times. I'm a premium member. Essentially what happens is that the fields that are emailed are not the same as the ones in the form. They seem to be out of sync.

Have you completely deleted the form and started from scratch (not simply manually deleting questions, but actually starting fresh in a new wizard)? This typically resolves this problem.
