I've created my form and it's being hosted by MyContact. But I'm not clear on how to go about customizing the form--for example, how do I add additional introductory text and possibly a banner/logo? Also, how do I remove the "email contact forms" text at the bottom of my form (I have a premium membership).

There is no way to further customize the form beyond what is available during the wizard. In addition, there is a bug that causes the 'email contact form' text which will be resolved after the next update (late August).
