I have had no problems with my contact form, however, it has been brought to my attention that when a form is filled out and submitted, the automatic e-mail comes in with all the information except the "From" line is blank. The subject line says "FormToEmail Comments" and the body of the e-mail has the information that was filled out. Our contact form is located on http://www.jpac.pacom.mil/ on the 'contact us' link.

I checked the ForToEmail coding but I am not too sure what to change. Your help is greatly appreciated.

This is most likely an issue with the email client that is being used to read the form submissions. Please point the form to a different email account read via a different client (such as gmail) and see if the error is still occurring.

When I went into the FormToEmail.php and changed the '$my_email' to a different e-mail adress I still received an "unknown sender" in the 'from' line. I even tried to change the subject line to display something other than the "FormToEmail Comments" but was unsucessful. Am I making the chages in the wrong place?

myContactForm.com does not have a FormToEmail.php file. You are most likely confusing us with another service. You should try myContactForm.com out:
