Good Day,
Typically when this problem occurs, it is due the incoming email server, email client, spam filters, or antivirus clients deleting the attachments from the message. If you look in the 'Message Center' at the particular message and it notes that there was an attachment on the message (it will list the actual file name), then it is being sent correctly.
Once you verify this, please also post the URLs of the forms, and I will look into it further.
