I cannot see the test page, has it been deleted?
Anyways, there should be no problem using two forms on the same page, as long as the html that we provide is not altered. The problem that is most likely occuring is that the html editor you are using (Dreamweaver, Frontpage, etc) is most likely seeing the two forms and then altering the html by doing things like changing that names of duplicate fields, and combining the <form> tags.
If it is not working, I would recommend getting fresh html from the control panel, and then placing it on your site using a text editor such as notepad. If you do not know how to do this, you may find instructions by logging into the control panel, clicking 'Help Center', and then clicking 'How Tos'.
