Hi

My form works brilliantly for
http://www.swchurchofchrist.co.za/

But for these three pages it just won't work.

http://www.swchurchofchrist.co.za/events.htm
http://www.swchurchofchrist.co.za/gallery.htm
http://www.swchurchofchrist.co.za/youth.htm

I have specified all the pages.

What could be the problem?

Double check Edit > Basic Information > Form Location.
