My form isnt working.

I have the HTML copy and pasted onto my website: phonedefense.com

but everytime someone puts in their email address and hits 'ENTER SITE' (its SUPPOSED to re-direct to shop.phonedefense.net

but instead its' giving this error message:

ERROR

This form is not being sent from the url where it is supposed to be located. If you are the owner of this form you need to ensure that the 'Form Location' is specified as flashquix.com/embeds/06a5a446bba94de78ec8f8455e1ba6c1. To change this, login to the site, click 'edit' next to the form's name, and then click the 'Basic Information' link to change the form's location. Currently you have the 'Form Location' specified as:
http://www.phonedefense.com

NOW THE PROBLEM is that when I change it to that...it gives the EXACT same message, except with a different URl with different numbers and letters. Even when I put the actual domain name, it does the same thing.

I'm not sure what else to do. Here is what's currently copy and pasted into the HTML box.

The problem is that Wix is embedding your form via an iframe. The source URL for the iframe appears to be dynamically generated each time it is loaded, thus making it tough to configure the form location setting. Instead of using a URL that changes with each load for the configuration, use a URL that includes a wildcard and will work each time. In your case, change the form location setting withing the configuration to:

flashquix.com/embeds/*

This should resolve the issue.
