Looking at the code you posted, it is clear that it is not the same as the code we've provided. In particular, all the parts that say readonly="true" are causing what you're experiencing and are not part of the original code we provided. This was most likely added to the code by you web host of authoring software to prevent you from adding forms to your site. You will need to contact them in order to prevent this from occuring.
