my form is on my website at www.JohnandSean.com ...there seems to be a problem with the first two lines (Subject line and email address). How do I correct this?

That is quite odd. Is it appearing this way in the form preview from the control panel? Please let me know, and I will look into the problem in further detail.

Yes, the problem with my email line and subject line appear in your preview as they do at www.johnandsean.com

It appears that in some way your form within our databases has been corrupted. I am going to look into the issue that might have caused this and try to fix the problem for our next update (end of November). In the mean time, you'll most likely be better off simply recreating the form.

So I tried redoing the form, and still I have the same trouble with the email and subject lines at the top. Can you do something about this?

Nick,

I have recreated my form a couple of times, but still cannot resolve the problem with the required subject line and the email line...see www.johnandsean.com click on the contact us button.

Can you fix this somehow, Please. I am trying to get my database updated using this form and I am a little embarassed to direct my clients to this form with the error in place.

Sean,

I logged into your account and changed some of the settings for the form. It turns out that the record for your form was fine, and it wasn't being caused by a bug as I had suspected. The problem had to do with the fact that the form was configured to be too narrow with too large of text boxes which was screwing up the was the web browser was rendering the form. Anyways, I've changed a few settings on the form 'Questionaire' and it looks pretty good now. All you should have to do is recopy and paste the new HTML to your site.
