I get this message when I try to submit the form:

To change this, login to the site, click 'edit' next to the form's name, and then click the 'Basic Information' link to change the form's location.


The form has been working for a year without any problems, and now it's not. I regenerated the code as someone suggested, but that didn't fix the problem. Meanwhile we aren't getting any of our applications.

I fixed this by adding a colon after the http://www.a-r-f.org. I'm not sure why that worked or why it happened in the first place. Just in case it happens to someone else.
