Good Day,

I'd try a few things.

1. Recopy and paste the code to your site. If something got inadvertidly changed with the field names / IDs, it can cause problems.

You could use our forms to do this. The email field is a prerequisite, although you could make it a hidden field with a predetermined address.

Our forms do not directly link to anything. If you go through the wizard, you can then copy and paste the code to any site you wish.
